Under the federal No Surprises Act, clients who do not have insurance or who choose not to use their insurance are entitled to receive a Good Faith Estimate of the expected cost of their care.
If you are paying privately, I will provide a Good Faith Estimate upon request and, when required, before scheduled services. Your estimate will outline the expected services and charges based on the information available at the time.
If your final bill is at least $400 more than your Good Faith Estimate, you may have the right to dispute the bill through the federal Patient-Provider Dispute Resolution process.
For more information about your rights under the No Surprises Act, visit the CMS No Surprises Act website.

If your insurance is not listed above therapy may still be covered by insurance as an out of network benefit. Out of network coverage varies widely. I encourage you to contact your insurance company about out-of-network benefits (including reimbursement, deductible, and number of sessions covered) prior to our first meeting. I can provide you with receipts (also called super-bills) to submit to your insurance company for reimbursement. You are responsible for the full session fee at the time of service.

I recommend calling your insurance company and asking the following:
Is Dr. Sarah Nelson, Ph.D., with Rose and Fern Counseling, an in-network provider?
What is my copay or coinsurance per session?
What is my annual deductible, and how much have I met so far?
If Dr. Nelson is out-of-network, what percentage of each session will my plan cover, and what will I be responsible for?
Are there any limits on the number of sessions I can have with Dr. Nelson?
-
Current session rates range from $250 to $275 depending on the service provided.
-
If you need to cancel your appointment, please provide at least 24 hours’ notice. Late cancellations with less than 24 hours’ notice are subject to a $100 fee for the first occurrence and the full session rate for subsequent occurrences. All no-shows are billed at the full session rate.
Insurance does not reimburse or cover late cancellation or no-show fees.
